Request a Duplicate ID Card

We are happy to provide our policyowners with a policy identification card. To order a free replacement card, click on the link above and fill out the online request.

Request a Duplicate PreScrip Card

We provide all of our Policyowners access to PreScrip - a discount prescription drug service administered by Emdeon. Your PreScrip card can be used at any pharmacy, enabling you to receive the lowest cost that day for any name brand or generic drugs.

Request a Duplicate Policy

If you are not able to locate your original policy, we will be happy to mail you a duplicate policy.

Name Change

Address Change

Having your correct address is very important to us in order to ensure that notices, checks and correspondence are communicated promptly.

Beneficiary Change
