To change ownership and control of a life insurance policy, print and complete an application for Contract Change and send it to us.

If the owner has passed away, the executor of the owner's estate must complete and sign an application for contract change naming the new owner. The executor must provide Letters Testamentary or Court Documents showing their authority.
